Administrative Plan 7/1/2025 Page 7-13 <br />Deceased Tenants Reports [Notice PIH 2012-4 and Notice PIH 2023-27] <br />The Deceased Tenant Report identifies residents that have been reported by the SSA as <br />deceased. SAHA is required to review the report at least quarterly. <br />SAHA Policy <br />SAHA will review the Deceased Tenants Report on a monthly basis. <br />When the Deceased Tenants Report identifies an individual as being deceased, PHAs must <br />immediately send a letter to the head of household or emergency contact person (if the head of <br />household is deceased and there is no other adult household member) to confirm the death of the <br />listed household member. The PHA must notify the owner in writing of the deceased head of <br />household. <br />PHAs may list the EOP as the last day of the month in which the death occurred. The landlord is <br />entitled to receive the full HAP amount for the month in which the tenant death occurred. <br />SAHA Policy <br />SAHA will list the EOP as the last day of the month in which the death occurred. The <br />landlord is entitled to receive the full HAP amount for the month in which the tenant <br />death occurred. <br />When the only remaining household member is the live-in aide, the live-in aide is not entitled or <br />eligible for any rental assistance or continued occupancy. The PHA may not designate the live-in <br />aide as the new head of household or change the relation code on the Form HUD-50058. <br />Other EIV Reports [Notice PIH 2023-27] <br />The PHA is required to review the Multiple Subsidy Report at least quarterly and the Failed EIV <br />Pre-Screening and Failed Verification (Failed SSA Identity Test) reports at least monthly. <br />Upfront Income Verification Using Non-HUD Systems <br />In addition to mandatory use of the EIV system, HUD encourages PHAs to utilize other upfront <br />verification sources. <br />SAHA Policy <br />SAHA will inform all applicants and participants of its use of other up-front income <br />verification sources, such as The Work Number (an automated verification system) and <br />state government databases, to validate tenant-reported income. <br />EXHIBIT 1
