LEVEL 4 VERIFICATION [Notice PIH 2023-27] <br />HUD identifies two types of Level 4 verification: written-third party verification from the source <br />and EIV + self-certification. <br />EIV + Self-Certification <br />EIV may be used as written third-party verification and may be used to calculate income if the <br />family agrees with the information in EIV and self-certifies that the amount is accurate and <br />representative of current income. This practice is known as EIV + self-certification. When <br />calculating income using this method, the PHA may use its discretion to determine which <br />method of calculation is reasonable: the last four quarters combined or an average of any number <br />of quarters. The family must be provided with the information from EIV. <br />SAHA Policy <br />If there are no reported changes to an income source, SAHA may use EIV + self- <br />certification as verification of employment income, provided the family agrees with the <br />amounts listed in EIV. <br />SAHA will use an average of the last two quarters of income listed in EIV to determine <br />income from employment. SAHA will provide the family with the information in EIV. <br />The family will be required to sign a self-certification stating that the amount listed in <br />EIV is accurate and representative of current income. If the family disagrees with using <br />only the last two quarters of income listed in EIV, because of the seasonal or otherwise <br />fluctuating nature of a particular family member’s employment, SAHA will permit the <br />family to sign a self-certification stating that the average of all four quarters of income <br />listed in EIV is accurate and representative of current annual income and use that amount <br />for calculating annual income. If the family disagrees and contends that the amount listed <br />in EIV is not reflective of current income, or if less than two quarters are available in <br />EIV, SAHA will use written third-party verification from the source as outlined below. <br />SAHA will not use this method of verification at new admission since EIV is not <br />available for applicant families or at interim reexamination since the income information <br />in EIV is not current. <br />Written Third-Party Verification from the Source <br />Written, third-party verification from the source is also known as “tenant-provided verification.” <br />In order to qualify as written-third party verification from the source, the documents must be <br />original or authentic and (generally) dated within 120 days of the date received by the PHA. For <br />fixed-income sources, a statement dated within the appropriate benefit year is acceptable <br />documentation. The PHA may use the verification obtained during an interim reexamination for <br />an annual reexamination if there have been no other changes to annual income since the interim <br />reexamination. Documents may be supplied by the family or received from a third-party source. <br />EXHIBIT 1
