Administrative Plan 7/1/2025 Page 7-15 <br />Examples of acceptable tenant-provided documents include, but are not limited to pay stubs, <br />payroll summary reports, employer notice or letters of hire and termination, SSA benefit <br />verification letters, bank statements, child support payment stubs, welfare benefit letters and/or <br />printouts, and unemployment monetary benefit notices. Income tax returns with corresponding <br />official tax forms and schedules attached and including third-party receipt of transmission for <br />income tax return filed (i.e., tax preparer’s transmittal receipt, summary of transmittal from <br />online source, etc.) are an acceptable form of written, third-party verification. <br />The PHA is required to obtain, at minimum, two current and consecutive pay stubs when <br />calculating income using third-party verification from the source. For new income sources or <br />when two pay stubs are not available, the PHA should determine income based on the <br />information from a traditional written, third-party verification form or the best available <br />information. <br />When the family disputes EIV-reported employment income, the PHA uses written third-party <br />verification. <br />When verification of assets is required, PHAs are required to obtain a minimum of one statement <br />that reflects the current balance of banking/financial accounts. <br />SAHA Policy <br />In general, SAHA will use third-party verification from the source in the following <br />circumstances: <br />At annual reexamination when EIV + self-certification is not used; <br />For all new admissions; and <br />For all interim reexaminations. <br />SAHA will not use this method if SAHA is able to use an income determination from a <br />means-tested federal assistance program or if SAHA uses EIV + self-certification as <br />outlined above. <br />In general, third-party documents provided by the family or the source must be dated <br />within 120 days of the date received by SAHA. However, for fixed-income sources, a <br />statement dated within the appropriate benefit year is acceptable documentation. <br />SAHA may reject documentation provided by the family if the document is not an <br />original, if the document appears to be forged, or if the document is altered, mutilated, or <br />illegible. If SAHA determines that third-party documents provided by the family are not <br />acceptable, SAHA will explain the reason to the family and request additional <br />documentation from the family or will use a lower form of verification such as a written <br />third-party verification form. <br />When verification of assets held by a banking or financial institution is required, SAHA <br />will obtain the three most current, consecutive months statements that reflect the current <br />balance of the account. <br />When pay stubs are used, SAHA will require the family to provide the two most current, <br />consecutive pay stubs.
