is trained on using our Freedom Life Map Assessment to measure deficits in our students'socio-emotional
development across six key domains, and monitoring the levels of response in order to adapt the goal plan that
guides each student's progress through our program.
Our goal is to create an environment of equity,so that these youth have equal access to the same opportunities as
their housed peers, which also includes the ability to participate in cultural and community events.Samples of
current enrichment activities include college visits(for high schoolers),movie and beach days, summer and sports
camps,book clubs,skills-based workshops,holiday events,birthday celebrations,and museum and aquarium
visits.
Our Santa Ana team of Case Managers are 100%bilingual,representing the demographics of the students we serve
in the district. And more importantly—they are well-versed in the myriad ways we help the students overcome the
cultural barriers confronting Hispanic students who are experiencing homelessness. Our solution is not
prescriptive, but tailored to fit the specific needs of each individual child no matter their gender, race or ethnicity.
As such,we pull from a"toolbox"of resources than can include any/all of the following:
-Individualized academic support&materials
-Weekly mentoring and tutoring(based on availability of tutors)
-Teacher coordination
-Parent involvement
-Personalized one-on-one Case management
-Monthly Life Skills Workshops(1 per month for 9 months)
-Definition of goals and a case plan
-Home visits/weekly check-ins
-McKinney Vento Coordination
-24/7 support
-Connections to needed community resources, such as housing, food and healthcare
-Career coaching, definition of future academic goals
-Collaboration with teachers,counselors,and administrators
-Assistance with college applications, and financial assistance(like FAFSA)
-Access to enrichment activities, such as those mentioned above
-24-hour access to our case managers,7 days a week
Outcomes:
1.Case Management:Provide holistic supportive services, centered around an intensive case management model,
to a minimum of 35 Santa Ana residents, adolescents and young adults(up to age 24)who are experiencing
barriers to education and employment due to homelessness.
2. High School Graduation:Our Project Hope Alliance high school seniors will have an on-time graduation rate of
80%or higher.Graduation rates are also tracked in our Salesforce database. In June 2024,94% of our HS seniors
graduated. This is contrasted with the average graduation rate for homeless youth in California of 72.4%.
3.Workshops:The goal of of our Life Skills Workshops is to provide youth with relevant skills in a manner that
builds upon previous skills.Topics may include themes such as the following:Goal Setting,Stress Management,
Healthy Relationships, and Online Safety. We also are embedding regular workshops for youth utilizing a
"roadmap"to guide students to a college education, coaching on FAFSA/Financial Aid and how to fund a college
experience,and Employment Readiness.We project 9 of our Santa youth(under this funding)will attend
Workshop sessions in the new school year.Overall, across all districts,we project serving 125 students through the
Life Skills Workshops program.
4. Post-HS Pilot Programming:Project Hope Alliance will launch a college and career readiness pilot program for 4
transitional age youth(of the 35 youth under this funding opportunity)that encompasses workshops focused on
life skills, navigational skills, postsecondary readiness, and career exploration.
Budget:
$44,647 for year 1 and$43,574 for year 2 for the support of four Case Managers dedicated to Santa Ana youth,
who qualify as homeless under the McKinney-Vento Act of 1987.
Total:$88,221 over a two-year term
EXHIBIT A
