City of Santa Ana <br /> 24-H HAP-10007 <br /> Page 2 of 31 <br /> EXHIBIT A <br /> individuals and families to maintain their permanent housing; and (2) continue to <br /> build on regional coordination developed through previous rounds of HHAP <br /> funding (See HSC section 50230 et seq.). <br /> HHAP funding shall: <br /> A. Be available to applicants for the purpose of reimbursement for planning <br /> and preparing the Regionally Coordinated Homelessness Action Plans <br /> required for the HHAP-5 application. <br /> B. Continue to build regional collaboration between Continuums of Care, <br /> counties, and cities in a given region, regardless of population, and <br /> ultimately be used to develop a unified regional response to <br /> homelessness by fostering robust regional collaboration and strategic <br /> partnerships aimed at fortifying the homeless services and housing <br /> delivery system. This should be achieved through the formulation of data- <br /> driven and cross-system plans designed to allocate resources in <br /> alignment with the state's priorities for homeless housing solutions. This <br /> means implementing strategies that create and sustain regional <br /> partnerships and prioritize permanent housing solutions. <br /> C. Ensure the long-term sustainability of housing and supportive services, by <br /> strategically pairing these funds with other local, state, and federal <br /> resources to effectively reduce and ultimately end homelessness. <br /> Grantees are encouraged to follow the guidance provided in "Putting the <br /> Funding Pieces Together: Guide to Strategic Uses of New and Recent <br /> State and Federal Funds to Prevent and End Homelessness". <br /> D. Demonstrate sufficient resources dedicated to long-term permanent <br /> housing solutions, including capital and operating costs. <br /> E. Demonstrate a commitment to address racial disproportionality in <br /> homeless populations and achieve equitable provision of services and <br /> outcomes for Black, Native and Indigenous, Latinx, Asian, Pacific <br /> Islanders and other People of Color who are disproportionately impacted <br /> by homelessness. <br /> F.
