services coordination for eligible Transitional Age Youth and Former Foster Youth, aged <br /> 18 through 24, who are experiencing homelessness or at risk of homelessness and who <br /> have been matched with a FYI voucher. <br /> E. On April 2, 2025, the Housing Authority issued Request for Proposal No. 25-057A <br /> ("RFP"), by which it sought a subcontractor to provide services coordination for <br /> Transitional Age Youth("TAY") and Former Foster Youth, aged 18 through 24, who are <br /> experiencing homelessness or at risk of homelessness and who have ben matched with a <br /> Foster Youth to Independence (FYI) voucher. Specifically, the subcontractor will deliver <br /> two core services: 1) housing navigation and landlord incentive services to assist the FYI <br /> voucher holder to lease a rental unit in the private market; and 2) independent living skill <br /> training and life skills support services to maintain stable housing and not return to <br /> homelessness. Per the RFP, "Priority shall be given to qualifying TAY youth at the Santa <br /> Ana Navigation Center(`Carnegie Homeless Shelter')." The RFP shall be incorporated by <br /> reference as though attached hereto in full. <br /> F. Subcontractor submitted a responsive proposal to the RFP that was selected by the <br /> Contractor. Subcontractor represents that it is able and willing to provide services <br /> described in the scope of work that was included in the RFP. <br /> G. Subcontractor has been made aware of the State HHAP-5 Contract and agrees to comply <br /> with the conditions of the State HHAP-5 Contract and the applicable State requirements <br /> governing the use of HHAP-5 grant funds. <br /> H. Subcontractor has been made aware of the HUD Obligation Letter and agrees to comply <br /> with the conditions of the award as outlined in PIH Notice 2022-14, "Implementation of <br /> the Federal Fiscal Year 2022 Funding Provisions for the Housing Choice Voucher <br /> Program" and the applicable Federal requirements governing the use of FYI CY 2022 <br /> Special Fees. <br /> I. In undertaking the performance of this Agreement, Subcontractor represents that it is <br /> knowledgeable in its field and that any services performed by Subcontractor under this <br /> Agreement will be performed in compliance with such standards as may reasonably be <br /> expected from a professional consulting firm in the field. Subcontractor also represents <br /> that it has the requisite qualifications, expertise, and experience to work within the <br /> provisions of the Program and is willing to use said HHAP grant funds and FYI Special <br /> Fees to operate within said Program, <br /> J. Contractor and Subcontractor have duly executed this HHAP Agreement for the <br /> expenditure and utilization of said HHAP and FYI funds. <br /> NOW THEREFORE, in consideration of the mutual and respective promises, and subject to the <br /> terms and conditions hereinafter set forth, the Parties agree as follows: <br /> Page 2 of 10 <br /> #2076148v2 <br />
