54 <br />i. The CDSS issues written notice to the contractor that the conditional <br />status has been cleared; <br />ii. The contractor is issued a clear contract; or <br />iii. The contract terminates according to its terms. <br />d. A contractor may request written verification from the CDSS that some of <br />the deficiencies have been corrected even if the contractor will not be <br />removed from conditional contract status. <br />e. While on conditional status, the contractor shall submit monthly fiscal and <br />attendance reports to CDFS. The first monthly report shall include a <br />current inventory of equipment purchased in whole or in part with contract <br />funds. <br />f. Contractors on conditional status are not eligible to apply for new or <br />additional funds. <br />g. Contractors on Conditional contract status continue to be subject to, <br />Termination, and Non-Renewal in accordance with paragraphs B, Review <br />of Contracts for Continued Funding; and F, Contract Termination <br />Procedure. <br />F. Contract Termination Procedure <br />1. Contractor's Termination for Convenience (5 CCR 18024) <br />a. A contractor may terminate the contract for any reason during the contract <br />term. <br />b. The contractor shall notify the CDSS of its intent to terminate the contract <br />at least ninety (90) calendar days prior to the date the contractor intends <br />to terminate the contract. <br />c. Within fifteen (15) days from the date the contractor notifies the CDSS of <br />its intent to terminate the contract, the contractor shall submit: <br />i. A current inventory of equipment purchased in whole or in part with <br />contract funds; and <br />ii. The names, addresses and telephone numbers of all families served <br />by the contract and all staff members funded by the contract. <br />iii. CFCC and Alternative Payment program contractors shall also submit <br />the names, addresses and telephone numbers of all providers of <br />subsidized services funded by the childcare and development contract.
