77 <br />on an application for services certifies that the criteria have been met and begins <br />the period of eligibility. (5 CCR 18078(t)) <br />Recipients of service means families and/or children enrolled in a childcare and <br />development program subsidized by the CDSS. (5 CCR 18078(u)) <br />Regional market rate ceilings means the maximum amount calculated by the <br />CDSS that providers in different regions of the state may be reimbursed for the <br />same type of childcare for the same age child in accordance with statutory <br />ceilings currently in effect. (5 CCR 18074.1(c)) <br />Restricted income means income which the donor designates may only be <br />expended for specific limited purposes that would be reimbursable according to <br />the contract. (5 CCR 18013) <br />Sectarian organization or sectarian childcare provider means any <br />organization or provider that engages in religious conduct or activity or that seeks <br />to maintain a religious identity in some or all of its functions. (45 CFR Part 98.2) <br />Self-Certification of Income means a declaration signed by the parent under <br />penalty of perjury identifying: (5 CCR 18078(v)) <br />a. To the extent known, the employer, address, and date of hire and stating <br />the rate and frequency of pay, total amount of income received for the <br />preceding month(s), the type of work performed, and the hours and days <br />worked; or <br />b. Families who are recipients of a means-tested program, pursuant to WIC <br />10271, the income declared on the application for the means-tested <br />government program, and a statement indicating the parent does not have <br />access to the application. <br />c. That the parent does not have income from employment and any source <br />of income used to support the family including non-wage income. <br />Service agreement is a legal instrument by which the agency purchases <br />services needed to carry out the childcare and development programs. Legal <br />instruments that include services which are clearly incidental to the agreement <br />are not considered service agreements. <br />Service delivery area means the community, geographic area, or political <br />subdivision in which the childcare and development services are to be provided <br />as specified in the Request for Applications. (5 CCR 18000(f)) <br />Site supervisor means a person, who, regardless of his or her title, has <br />operational program responsibility for a childcare and development program at a <br />single site. A site supervisor shall hold a permit issued by the Commission on <br />Teacher Credentialing that authorizes supervision of a childcare and
