Effective July 1, 1994, the City will contribute into the <br /> Santa Ana Police Officer Association Medical Insurance Trust Fund <br /> a maximum of $385 . 00 per month for each eligible employee enrolled <br /> in health insurance plans provided by the Association for its <br /> bargaining unit members and their eligible dependents . Any <br /> contribution necessary to maintain benefits under said health <br /> insurance plan in excess of $385 . 00 per month shall be borne <br /> entirely by the Association and/or the employees . <br /> B . Dental Insurance. Effective July 1, 1993 , the City <br /> will contribute into the Santa Ana Police Officer Association <br /> Medical Insurance Trust Fund a maximum of thirty dollars ($30 . 00) <br /> per month for each affected employee enrolled in dental insurance <br /> plans provided by the Association for its bargaining unit members <br /> and their eligible dependents . Any contributions necessary to <br /> maintain benefits under said dental plans in excess of $30 . 00 per <br /> month shall be borne entirely by the Association and/or the <br /> enrollees . <br /> C. Long-Term Disability (L.T.D. ) Insurance . The City <br /> shall continue to contribute to the Santa Ana Police Officer <br /> Medical Insurance Trust Fund an amount toward the payment of <br /> premiums for each employee who is enrolled in the long-term <br /> disability insurance plan provided by the Association for employees <br /> covered by this Agreement in accordance with the following <br /> schedule : <br /> 1 . With respect to sworn, "safety-member" police <br /> personnel, up to a maximum of sixty-eight dollars ($68 . 00) per <br /> month per enrollee . <br /> 2 . With respect to non-sworn, "miscellaneous-member" <br /> personnel, up to a maximum of sixty-four dollars ($64 . 00) per month <br /> per enrollee . <br /> Any contribution necessary to maintain benefits <br /> under such long-term disability insurance plan provided by the <br /> Association for covered employees in excess of the amounts set <br /> forth as above shall be borne by the Association and/or the <br /> enrollees . <br /> Section 2 . Life Insurance . The City shall maintain in effect <br /> for the term of this Agreement its existing life insurance plan for <br /> employees covered by this Agreement on the same basis as said plan <br /> was offered to employees as of December 31, 1988 except the term <br /> life insurance coverage for each affected employee will be in the <br /> amount of twenty thousand dollars ($20 , 000) , plus twenty thousand <br /> dollars ($20, 000) accidental death and dismemberment (AD + D) <br /> coverage, at no cost to the employee . <br /> Section 3 . Employees on unpaid leave of more than 15 calendar <br /> days shall not receive the City' s contribution toward any insurance <br /> 31 <br />
