An employee returning to duty with the City shall inform <br /> the department head and Chief Personnel Officer of his or her <br /> intention at least thirty (30) calendar days prior to the <br /> expiration of the six (6) months period or shorter period if the <br /> full six (6) months is not taken. Upon receipt of such notice, the <br /> department head will take steps necessary to restore the employee <br /> to his or her former position. <br /> Section 9 . Administrative Leave . <br /> The City Manager is authorized to grant, at his discretion, <br /> administrative leave with or without pay for permanent employees <br /> if, in his opinion, such a leave is in the interest of the City. <br /> Section 10 . Industrial Leave . <br /> A. Each "safety member" employee covered by the <br /> provisions of Labor Code Section 4850 who is compelled to be absent <br /> from duty because of an illness or injury covered by the State of <br /> California Workers' Compensation Insurance and Safety Act shall, in <br /> lieu of temporary disability compensation payable under the <br /> aforementioned Act, continue to be paid his or her normal salary <br /> and accrue other benefits in accordance with the provisions of <br /> Labor Code Section 4850 . <br /> B. Any period of time during which an employee is <br /> required to be absent from his or her position by reason of an <br /> industrial injury or industrial illness for which he or she is <br /> entitled to receive compensation shall not be considered a break in <br /> continuous service for the purpose of his or her right to salary <br /> adjustment or to the accrual of vacation and seniority. <br /> ARTICLE XII - EMPLOYEE INSURANCE <br /> Section 1 . During the term of this Agreement the City will <br /> contribute toward the payment of premiums for health, dental and <br /> long-term disability insurance plans administered by the <br /> Association for the benefit of the employee represented by the <br /> Association as follows : <br /> A. Health Insurance . <br /> Effective July 1, 1993 , the City will contribute into the <br /> Santa Ana Police Officer Association Medical Insurance Trust Fund <br /> a maximum of $360 . 00 per month for each eligible employee enrolled <br /> in health insurance plans provided by the Association for its <br /> bargaining unit members and their eligible dependents . Any <br /> contribution necessary to maintain benefits under said health <br /> insurance plan in excess of $360 . 00 per month shall be borne <br /> entirely by the Association and/or the employees . <br /> 30 <br />
