The additional $49.25 represents SOo of the amount of the <br /> 1990-91 increase in the total monthly premium for the most expensive <br /> family coverage among HMO plans provided by the City for its non-POA <br /> employees. <br /> b. Effective October 1, 1991, the City will <br /> increase its contribution for such family coverage by an amount equal to <br /> SOo of the dollar increase in the City's premium for the most expensive <br /> family coverage among [-NO plans provided by the City for its non-POA <br /> employees. <br /> c. Effective October 1, 1992, the City will <br /> increase its contribution for such family coverage by an amount equal to <br /> SOo of the dollar increase in the City's premium for the most expensive <br /> family coverage among HMO plans provided by the City for its non-POA <br /> employees. <br /> B. Dental Insurance. The City will continue to contribute <br /> up to a maximum of twenty dollars ($20.00) per month for each affected <br /> employee enrolled in dental insurance plans provided by the Association <br /> for its bargaining unit members and their eligible dependents. Any <br /> contributions necessary to maintain benefits under said dental plans in <br /> excess of $20.00 per month shall be borne entirely by the Association <br /> and/or the enrollees. <br /> C. Long-Term Disability (L.T.D.) Insurance. The City shall <br /> continue to contribute an amount toward the payment of premiums for each <br /> employee who is enrolled in the long-term disability insurance plan <br /> provided by the Association for employees covered by this Agreement in <br /> accordance with the following schedule: <br /> 1. With respect to sworn, "safety-member" police <br /> personnel, up to a maximum of sixty-eight dollars ($68.00) per month per <br /> enrollee. <br /> 2. With respect to non-sworn, "miscellaneous-member" <br /> personnel, up to a maximum of sixty-four dollars ($64.00) per month per <br /> enrollee. <br /> Any contribution necessary to maintain benefits <br /> under such long-term disability insurance plan provided by the <br /> Association for covered employees in excess of the amounts set forth as <br /> above shall be borne by the Association and/or the enrollees. <br /> Section 2. Life Insurance. The City shall maintain in effect for <br /> the term of this Agreement its existing life insurance plan for employees <br /> covered by this Agreement on the same basis as said plan was offered to <br /> employees as of December 31, 1988 except the term life insurance coverage <br /> for each affected employee will be in the amount of twenty thousand <br /> dollars ($20,000) , plus twenty thousand dollars ($20,000) accidental <br /> death and dismemberment (AD + D) coverage, at no cost to the employee. <br /> -27- <br />
