in accordance with the following schedule : <br /> i . With respect to sworn, "safety-member" police <br /> personnel, up to a maximum of sixty-eight <br /> dollars ($68 . 00) per month per enrollee . <br /> 2 . With respect to non-sworn, "miscellaneous- <br /> member" personnel, up to a maximum of sixty- <br /> four dollars ($64 . 00) per month per enrollee. <br /> Any contribution necessary to maintain benefits under <br /> such long-term disability insurance plan provided by the <br /> Association for covered employees in excess of the <br /> amounts set forth as above shall be borne by the <br /> Association and/or the enrollees . <br /> D. Life Insurance . The City shall maintain in effect <br /> for the term of this Agreement its existing life <br /> insurance plan for employees covered by this <br /> Agreement on the same basis as said plan was <br /> offered to employees as of December 31, 1988 except <br /> the term life insurance coverage for each affected <br /> employee will be in the amount of twenty thousand <br /> dollars ($20, 000) , plus twenty thousand dollars <br /> ($20, 000) accidental death and dismemberment (AD + <br /> D) coverage, at no cost to the employee . <br /> 12 . 2 Retiree Health Insurance Plan. Members retiring on or <br /> after July 1, 1997, will be provided health insurance <br /> premium reduction assistance . <br /> For retirees out of the health insurance service area, <br /> the City shall require verification of health insurance <br /> premiums and will pay said retirees directly for their <br /> health insurance. However, in no case will payment <br /> exceed actual premium cost . <br /> The Association agrees to a no-interest loan to the <br /> Retiree Health Insurance Plan of $300, 000 for immediate <br /> implementation of this plan. The loan is repayable at <br /> the expiration of the Agreement, and it will be repaid <br /> from the one-half percent ( . 5%) collected from the active <br /> employees' salary deduction. <br /> In order to continue funding the plan, effective July 1, <br /> 1998 , bargaining unit members will contribute one-half <br /> percent ( .5%) of the 1998-99 salary increase and one-half <br /> percent ( . 5%) of the 1999-00 salary increase. These <br /> amounts will be contributed in the form of a monthly <br /> salary deduction from each employee represented by the <br /> Police Officers Association. <br /> 46 <br />
