ARTICLE XII <br /> 12 . 0 EMPLOYEE INSURANCE <br /> 12 . 1 During the term of this Agreement the City will <br /> contribute toward the payment of premiums for health, <br /> dental and long-term disability insurance plans <br /> administered by the Association for the benefit of the <br /> employees represented by the Association as follows : <br /> A. Health Insurance . Effective November 1, 1996, the <br /> City will contribute into the Santa Ana Police <br /> Officer Association Medical Insurance Trust Fund a <br /> maximum of $400 . 00 per month for each eligible <br /> employee enrolled in health insurance plans <br /> provided by the Association for its bargaining unit <br /> members and their eligible dependents . Any <br /> contribution necessary to maintain benefits under <br /> said health insurance plan in excess of $400 . 00 per <br /> month shall be borne entirely by the Association <br /> and/or the employees . Further, all bargaining unit <br /> employees and the Association shall hold the City <br /> harmless, defend and indemnify the City for any <br /> claims regarding the administration of , or the <br /> payment of, claims under any Association-sponsored <br /> medical plan (s) . <br /> If any other City bargaining unit receives an <br /> increase in contribution toward health insurance <br /> benefits during the term of this Agreement, the <br /> employees covered by this Agreement will receive an <br /> equivalent amount . <br /> B . Dental Insurance . The City will contribute into the <br /> Santa Ana Police Officer Association Medical <br /> Insurance Trust Fund a maximum of thirty dollars <br /> ($30 . 00) per month for each affected employee <br /> enrolled in dental insurance plans provided by the <br /> Association for its bargaining unit members and <br /> their eligible dependents . Any contributions <br /> necessary to maintain benefits under said dental <br /> plans in excess of $30 . 00 per month shall be borne <br /> entirely by the Association and/or the enrollees . <br /> C. Long-Term Disability (L.T.D. ) Insurance . The City <br /> shall continue to contribute to the Santa Ana <br /> Police Officer Medical Insurance Trust Fund an <br /> amount toward the payment of premiums for each <br /> employee who is enrolled in the long-term <br /> disability insurance plan provided by the <br /> Association for employees covered by this Agreement <br /> 45 <br />
