STATEWIDE SANITARY SEWER SYSTEMS GENERAL ORDER 2022-0103-DWQ <br />4. Spill cause(s) (for example, root intrusion, grease deposition, etc.); <br />5. System failure location (for example, main, lateral, pump station, etc.); <br />6. Description of the pipe material, and estimated age of the pipe material, at the failure <br />location; <br />7. Description of the impact of the spill; <br />8. Whether or not the spill was associated with a storm event; <br />9. Description of spill response activities including description of immediate spill <br />containment and cleanup efforts; <br />10. Description of spill corrective action, including steps planned or taken to reduce, <br />eliminate, and prevent reoccurrence of the spill, and a schedule of major milestones <br />for those steps; <br />11. Spill response completion date; <br />12. Detailed narrative of investigation and investigation findings of cause of spill; <br />13. Reasons for an ongoing investigation (as applicable) and the expected date of <br />completion; <br />14. Name and type of receiving water body(s); <br />15. Description of the water body(s), including but not limited to: <br />o Observed impacts on aquatic life, <br />o Public closure, restricted public access, temporary restricted use, and/or posted <br />health warnings due to spill, <br />o Responsible entity for closing/restricting use of water body, and <br />o Number of days closed/restricted as a result of the spill. <br />16. Whether or not the spill was located within 1,000 feet of a municipal surface water <br />intake; and <br />17. If water quality samples were collected, identify sample locations and the <br />parameters the water quality samples were analyzed for. If no samples were taken, <br />Not Applicable shall be selected. <br />3.1.3. Spill Technical Report for Individual Category 1 Spill in which 50,000 Gallons or <br />Greater Discharged into a Surface Water <br />For any spill in which 50,000 gallons or greater discharged into a surface water, <br />within 45 calendar days of the spill end date, the Enrollee shall submit a Spill <br />Technical Report to the online CIWQS Sanitary Sewer System Database. The Spill <br />Technical Report, at minimum, must include the following information: <br />1.
