STATEWIDE SANITARY SEWER SYSTEMS GENERAL ORDER 2022-0103-DWQ <br />o Photographs illustrating the spill origin, <br />conveyance system entrance and exit, <br />conditions; <br />the extent and reach of the spill, drainage <br />receiving water, and post -cleanup site <br />o Diagram showing the spill failure point, appearance point(s), the spill flow path, <br />and ultimate destinations; <br />o Detailed description of the methodology employed, and available data used to <br />calculate the discharge volume and, if applicable, the recovered spill volume; <br />o Detailed description of the spill cause(s); <br />o Description of the pipe material, and estimated age of the pipe material, at the <br />failure location; <br />o Description of the impact of the spill; <br />o Copy of original field crew records used to document the spill; and <br />o Historical maintenance records for the failure location. <br />2. Enrollee's response to the spill: <br />o Chronological narrative description of all actions taken by the Enrollee to <br />terminate the spill; <br />o Explanation of how the Sewer System Management Plan Spill Emergency <br />Response Plan was implemented to respond to and mitigate the spill; and <br />o Final corrective action(s) completed and a schedule for planned corrective <br />actions, including: <br />■ Local regulatory enforcement action taken against an illicit discharge in <br />response to this spill, as applicable, <br />■ Identifiable system modifications, and operation and maintenance program <br />modifications needed to prevent repeated spill occurrences, and <br />■ Necessary modifications to the Emergency Spill Response Plan to <br />incorporate lessons learned in responding to and mitigating the spill. <br />3. Water Quality Monitoring, including at minimum: <br />o Description of all water quality sampling activities conducted; <br />o List of pollutant and parameters monitored, sampled and analyzed; as required in <br />section 2.3 (Receiving Water Monitoring) of this Attachment; <br />o Laboratory results, including laboratory reports; <br />o Detailed location map illustrating all water quality sampling points; and <br />o Other regulatory agencies receiving sample results (if applicable). <br />4.
