Santa Ana Police Department Homicide Sergeant M. Sarabia #2565 notified me of an in-custody death that <br />had occurred at the Santa Ana Jail. Sergeant Sarabia requested that I respond to the Santa Ana Police <br />Department in order to assist with the investigation. <br />1345 hours: <br />I responded to the Santa Ana Police Department and was provided a preliminary briefing of the incident by <br />Sergeant Sarabia. Present at this briefing were personnel from the Santa Ana Police Homicide Section and <br />Orange County District Attorney Special Assignments Unit. <br />Sergeant Sarabia informed us that this morning, at approximately 1125 hours, Santa Ana Jail Supervisor M. <br />Monreal #3181 contacted the Santa Ana Police Department Watch Commander and reported that an <br />inmate was down as a result of a suicide attempt. Orange County Fire Authority Medics were responding to <br />the jail and a police unit was being requested to follow the medics with the inmate to the hospital. At <br />approximately 1154 hours, the inmate was pronounced deceased and was not being transported to the <br />hospital. <br />The deceased inmate was subsequently identified as Darryle Marcell Samuel (DOB Samuel <br />was being housed in cell ³C17´by himself. Samuel was a federal US Marshals inmate who was presently <br />acting as an informant on a homicide case. <br />1415 hours: <br />I responded to the Santa Ana Jail with personnel from the Santa Ana Police Homicide Section and Orange <br />County District Attorney Special Assignments Unit. We were subsequently escorted to 4th Floor Module 4 <br />C. Supervisor Monreal then conducted a briefing regarding the circumstances of the incident. Present at <br />this briefing were personnel from the Santa Ana Police Department, Orange County District Attorney ¶s <br />Office, and Orange County Sheriff-Coroner¶s Department. <br />Supervisor Monreal informed us that this morning, at approximately 1119 hours, Santa Ana Jail Correctional <br />Officer E. Gonzalez #3366 radioed to jail staff for medics and additional correctional staff to respond to <br />Module 4 C. This Module was comprised of thirty-two cells capable of holding two inmates each and was <br />designated as a protective custody unit, with inmates who were either cooperators with law enforcement or <br />charged with sex-related offenses. <br />Supervisor Monreal indicated that she responded to the Module at approximately 1120 hours. The <br />assistance was being requested by Correctional Officer Gonzalez for the inmate being housed in Cell 17. <br />Correctional Officers Gonzalez and R. Zavala #2450 were in the cell attempting to render medical aid to the <br />inmate in Cell 17. <br />The inmate in Cell 17 was identified as Darryle Marcell Samuel (DOB Samuel had been in <br />custody at the Santa Ana Jail since May 24th, 2021. Samuel was on the floor of the cell adjacent to the <br />toilet. Correctional Officers Gonzalez and Zavala rendered aid to the inmate but he was unresponsive. At <br />approximately 1130 hours, Orange County Fire Authority (OCFA) personnel from Station 76 arrived on <br />scene and assumed medical aid of Samuel. At approximately 1141 hours, Emergency Medical Technicians <br />and Paramedics arrived on scene to assist in rendering aid to Samuel. At approximately 1150 hours, OCFA <br />Fire Captain Robert Richardson #5760 informed Supervisor Monreal that Samuel was deceased and would <br />not be transported from the scene. <br />Supervisor Monreal indicated that at approximately 1152 hours the doors to Cell 17 were closed and the cell <br />was secured with no additional persons accessing the cell. <br />Supervisor Monreal advised us that inmate Samuel came out of his cell to the day room of the Module this <br />morning at approximately 0714 hours. Samuel watched television and socialized with other inmates and <br />returned to his cell at approximately 0914 hours. At approximately 0930 hours, Correctional Officer S. <br />Tolone #3687 conducted a welfare check on Samuel¶s cell and documented the check in a log. At <br />approximately 1119 hours, two inmate workers named ³ ´and ³ ´were going from cell to cell <br />cleaning and providing deliveries and then alerted custody staff that there was an inmate hanging in the cell. <br />Santa Ana PD 2021-13541 Report by McClaskey, Paul 3110 17 of 19
