The deceased inmate was subsequently identified as Darryle Marcell Samuel (DOB <br />  6DPXHO ZDV EHLQJ KRXVHG LQ FHOO ³C17´ E\ KLPVHOI 6DPXHO ZDV D IHGHUDO 86 <br />0DUVKDOV LQPDWH ZKR ZDV SUHVHQWO\ DFWLQJ DV DQ LQIRUPDQW RQ D KRPLFLGH FDVH <br />I was assigned to document the scene and was paired to work with Orange County District <br />Attorney¶s Office Special Assignments Investigator Dave Melnyk. <br />1415 hours: <br />I responded to the Santa Ana Jail with personnel from the Santa Ana Police Homicide <br />6HFWLRQ DQG 2UDQJH &RXQW\ 'LVWULFW $WWRUQH\ 6SHFLDO $VVLJQPHQWV 8QLW :H ZHUH <br />subsequently escorted to 4th )ORRU 0RGXOH  & 6XSHUYLVRU 0RQUHDO WKHQ FRQGXFWHG D <br />EULHILQJ UHJDUGLQJ WKH FLUFXPVWDQFHV RI WKH LQFLGHQW 3UHVHQW DW WKLV EULHILQJ ZHUH SHUVRQQHO <br />from the Santa Ana Police Department, Orange County District Attorney¶s Office, and <br />Orange County Sheriff-Coroner¶s Department. <br />Supervisor Monreal informed us that this morning, at approximately 1119 hours, Santa Ana <br />Jail Correctional Officer E. Gonzalez #3366 radioed to jail staff for medics and additional <br />FRUUHFWLRQDO VWDII WR UHVSRQG WR 0RGXOH  & 7KLV 0RGXOH ZDV FRPSULVHG RI WKLUW\WZR FHOOV <br />capable of holding two inmates each and was designated as a protective custody unit, with <br />inmates who were either cooperators with law enforcement or charged with sex-related <br />offenses. <br />Supervisor Monreal indicated that she responded to the Module at approximately 1120 <br />KRXUV 7KH DVVLVWDQFH ZDV EHLQJ UHTXHVWHG E\ &RUUHFWLRQDO 2IILFHU *RQ]DOH] IRU WKH LQPDWH <br />EHLQJ KRXVHG LQ &HOO  &RUUHFWLRQDO 2IILFHUV *RQ]DOH] DQG 5 =DYDOD  ZHUH LQ WKH <br />cell attempting to render medical aid to the inmate in Cell 17. <br />7KH LQPDWH LQ &HOO  ZDV LGHQWLILHG DV 'DUU\OH 0DUFHOO 6DPXHO '2%   6DPXHO <br />had been in custody at the Santa Ana Jail since May 24th  6DPXHO ZDV RQ WKH IORRU RI <br />WKH FHOO DGMDFHQW WR WKH WRLOHW &RUUHFWLRQDO 2IILFHUV *RQ]DOH] DQG =DYDOD UHQGHUHG DLG WR WKH <br />LQPDWH EXW KH ZDV XQUHVSRQVLYH $W DSSUR[LPDWHO\  KRXUV 2UDQJH &RXQW\ )LUH <br />Authority (OCFA) personnel from Station 76 arrived on scene and assumed medical aid of <br />6DPXHO $W DSSUR[LPDWHO\  KRXUV (PHUJHQF\ 0HGLFDO 7HFKQLFLDQV DQG 3DUDPHGLFV <br />DUULYHG RQ VFHQH WR DVVLVW LQ UHQGHULQJ DLG WR 6DPXHO $W DSSUR[LPDWHO\  KRXUV 2&)$ <br />Fire Captain Robert Richardson #5760 informed Supervisor Monreal that Samuel was <br />deceased and would not be transported from the scene. <br />Supervisor Monreal indicated that at approximately 1152 hours the doors to Cell 17 were <br />closed and the cell was secured with no additional persons accessing the cell. <br />Supervisor Monreal advised us that inmate Samuel came out of his cell to the day room of <br />WKH 0RGXOH WKLV PRUQLQJ DW DSSUR[LPDWHO\  KRXUV 6DPXHO ZDWFKHG WHOHYLVLRQ DQG <br />VRFLDOL]HG ZLWK RWKHU LQPDWHV DQG UHWXUQHG WR KLV FHOO DW DSSUR[LPDWHO\  KRXUV $W <br />approximately 0930 hours, Correctional Officer S. Tolone #3687 conducted a welfare check <br />on Samuel¶V FHOO DQG GRFXPHQWHG WKH FKHFN LQ D ORJ $W DSSUR[LPDWHO\  KRXUV WZR <br />inmate workers named ³ ´and ³ ´were going from cell to cell cleaning and <br />providing deliveries and then alerted custody staff that there was an inmate hanging in the <br />Santa Ana PD 2021-13541 Report by Gibbons, Michael 2990 4 of 6
