S.A. 12-0026 <br />Investigator: FRANK CARDENAS Approved By: <br />Date of Report: 10/30/12 Date: 11/14/12 <br />Page 1 of 4 <br /> <br /> <br />CASE NAME: GUILLEN, JAIME LOPEZ <br />CLASSIFICATION: OFFICER INVOLVED SHOOTING (NON-FATAL) <br />TYPE: CANVASS <br />AGENCY: SANTA ANA POLICE DEPARTMENT <br />On October 22, 2012, at approximately 2241 hours, Orange County District Attorney <br />Supervising Investigator RON SEMAN contacted me regarding an officer involved shooting that <br />involved the Santa Ana Police Department. SEMAN requested I respond to 525 West Walnut <br />Street in the City of Santa Ana and assist with the investigation. <br />At approximately 2330 hours, I arrived at the command center location and met with <br />investigators from the Orange County District Attorney’s Office and Santa Ana Police <br />Department who were present to assist in the investigation. Orange County District Attorney <br />Investigator PAUL CARVO requested I conduct a canvass interview north of the crime scene <br />which was located in the alley/ driveway behind 210 South Birch Street. I was teamed with <br />Santa Ana Police Department, Detective LEO RODRIGUEZ. <br />At approximately 0055 hours, Detective LEO RODRIGUEZ and I conducted canvass <br />interviews on Walnut and Birch north of the crime scene. The interviews were audio digitally <br />recorded.
