SA 12-026 <br />Investigator: ANDREW ROSS Approved By: <br />Date of Report: 10/23/12 Date: 10/24/12 <br />Page 1 of 3 <br /> <br /> <br />CASE NAME: GUILLEN, JAIME LOPEZ <br />CLASSIFICATION: OFFICER INVOLVED SHOOTING (NON-FATAL) <br />TYPE: INITIAL RESPONSE/HOSPITAL <br />AGENCY: SANTA ANA POLICE DEPARTMENT <br />On October 22, 2012, at approximately 2230 hours, Orange County District Attorney <br />(OCDA) Supervising Investigator RON SEMAN contacted me regarding the Santa Ana Police <br />Department (SAPD) officer involved shooting of JAIME LOPEZ GUILLEN. SEMAN requested I <br />respond to Western Medical Center – Santa Ana (WMCSA), located at 1001 North Tustin <br />Avenue, Santa Ana, to handle the hospital investigation responsibilities. <br />At approximately 2330 hours, I arrived at WMCSA emergency room. GUILLEN was <br />confined to a hospital bed, handcuffed to the bed rail, inside Trauma Bay 1 (T-1), which was <br />located in the emergency room department. GUILLEN was conscious and was engaged in <br />conversation with the trauma nurse, RN . SAPD Officer DON KOSAKOWSKI <br />was also present inside the trauma room with GUILLEN. <br />On October 23, 2012, at approximately 0010 hours, SAPD Detective DEAN FULCHER <br />arrived at WMCSA to assist with the investigation. <br />At approximately 0030 hours, GUILLEN was transferred from the emergency room <br />department to room 718, which was a single-bed custody unit located on the seventh floor of <br />the hospital. <br />ORANGE COUNTY DISTRICT ATTORNEY’S OFFICE <br />BUREAU OF INVESTIGATION <br /> <br />SUPPLEMENTAL REPORT
