SA 12-026 <br />Investigator: ANDREW ROSS Approved By: <br />Date of Report: 10/23/12 Date: 10/24/12 <br />Page 2 of 3 <br />At approximately 0105 hours, GUILLEN voluntarily signed an “Authorization for Release <br />of Health Information” form, allowing the OCDA’s office to obtain a copy of GUILLEN’s medical <br />records and pre transfusion/pre admitting blood samples. <br />At approximately 0120 hours Orange County Crime Lab Forensic Specialist LINDSEY <br />OLSON arrived at room 718. OLSON took 10 photographs of GUILLEN’s injuries. GUILLEN <br />refused to provide a buccal swab. <br />At approximately 0220 hours, OLSON concluded her investigation and left the scene. <br />At approximately 0325 hours, OLSON returned to the WMCSA and took custody of <br />GUILLEN’s property, which consisted of one pair of boxer shorts. The boxer shorts had been cut <br />off GUILLEN by medical personnel and were blood stained. After OLSON took possession of the <br />property, she left the scene. <br />At approximately 0330 hours, SAPD Detective FULCHER and I concluded our <br />investigation and left the scene. <br />At approximately 1045 hours, I responded back to WMCSA and provided a copy of <br />GUILLEN’s signed “Authorization for Release of Health Information” form to the WMCSA <br />records department, requesting all documentation related to GUILLEN’s treatment. <br />At approximately 1100 hours, I provided a copy of GUILLEN’s signed “Authorization for <br />Release of Health Information” form to the WMCSA blood laboratory and took possession of <br />GUILLEN’s pre transfusion/pre admitting blood. <br />At approximately 1125 hours, I booked GUILLEN’s pre admitting/pre transfusion blood <br />samples into evidence at the OCSD property and evidence unit. <br />I conducted the following digital recorded interviews: <br />• Registered Nurse
