Cal OES shall provide copies of each course provided: Daily sign-in sheet, <br /> evaluations and certificates. <br /> 5. AGENCY RESPONSIBILITIES <br /> A. The Agency shall designate a point of contact, to whom all communication may be <br /> addressed, with the authority to act on all aspects of the services. This person will review <br /> the SOW and associated documents to ensure understanding of the responsibilities of <br /> both parties. <br /> B. The Agency agrees to provide Cal OES the following: <br /> 1. The Agency must provide locations for course(s) a minimum of 60 days prior to <br /> the course(s) start date. <br /> ii. The Agency must provide a point of contact for facility usage and classroom <br /> sefup for the scheduled course(s). <br /> iii. The Agency must provide course rosters to the Cal OES Contract Manager at a <br /> minimum of two weeks or 14 days, prior to the course(s) start date. <br /> iv. The Agency must provide a minimum of 30 days' notice prior to the agreed <br /> training course(s) start date if the course(s) shall be cancelled. Notice shall be <br /> provided to and received by the Cal OES Contract Manager. Failure to provide <br /> such notice will result in administration fees associated with the course(s). <br /> C. The Agency agrees to verify charges that will be provided by CSTI Financial <br /> Department. <br /> D. The Agency is responsible for the payment as outlined in Exhibit B, Budget Detail and <br /> Payment Provisions and B-1, Cost Sheet. <br /> E, The Agency agrees to indemnify, defend, and save harmless Cal OES, its officers, <br /> agents, and employees from any and all claims and losses accruing or resulting in <br /> connection with the performance of this agreement, and from any and all claims and <br /> losses accruing or resulting to any person, firm, or corporation who may be injured or <br /> damaged by the Agency in the performance on this Agreement. <br /> 6. PROBLEM ESCALATION <br /> The parties acknowledge and agree that certain technical and project related <br /> problems or issues may arise, and that such matters shall be brought to Cal DES's <br /> attention. There may be instances where the severity of the problem(s) justifies escalated <br /> reporting. To this extent, the Agency will determine the level of severity and notify the <br />
