2010/11-2014-15 CITY OF SANTA ANA CONSOLIDATED PLAN I SUBSTANTIAL AMENDMENT TO THE <br />2011/12 ACTION PLAN <br />POLICIES AND PROCEDURES FOR COORDINATION AMONG EMERGENCY SHELTER PROVIDERS, <br />ESSENTIAL SERVICE PROVIDERS, HOMELESSNESS PREVENTION AND RAPID RE-HOUSING ASSISTANCE <br />PROVIDERS, OTHER HOMELESS ASSISTANCE PROVIDERS, AND MAINSTREAM SERVICE AND HOUSING <br />PROVIDERS. <br />Periodic meetings will be held with the City and sub recipients to coordinate services. <br />Additionally, sub recipients will be in communication with each other when an individual seeks <br />service from two or more ESG funded sub recipients. <br />POLICIES AND PROCEDURES FOR DETERMINING AND PRIORITIZING WHICH ELIGIBLE FAMILIES AND <br />INDIVIDUALS WILL RECEIVE HOMELESSNESS PREVENTION ASSISTANCE AND WHICH ELIGIBLE FAMILIES <br />AND INDIVIDUALS WILL RECEIVE RAPID RE-HOUSING ASSISTANCE. ALL PROGRAM PARTICIPANTS ARE <br />ALREADY LITERALLY HOMELESS AND WILL ONLY BE USING RAPID REHOUSING ASSISTANCE. <br />The City has provided funds to varying sub recipients, each with their own mission. Therefore <br />each recipient will determine a priority based on the organization's mission. An overall priority <br />will be given to those families and individuals that at the time of initial consultation <br />demonstrate an ability to be self-sustaining. Due to the limited funds organizations will be <br />taking additional efforts during the evaluation process to determine which families and <br />individuals would require the least amount of resources in order to assist the greatest number <br />of people possible. <br />STANDARDS FOR DETERMINING THE SHARE OF RENT AND UTILITIES COSTS THAT EACH PROGRAM <br />PARTICIPANT MUST PAY, IF ANY, WHILE RECEIVING HOMELESSNESS PREVENTION OR RAPID <br />REHOUSING ASSISTANCE. <br />Sub recipients will perform a gap analysis to determine what will be the initial assistance to the <br />client. Additional assistance after that will be on a declining basis. Additionally, case-by-case <br />analysis will be performed to determine if an adjustment to the assistance needs to be made. <br />STANDARDS FOR DETERMINING HOW LONG A PARTICULAR PROGRAM PARTICIPANT WILL BE <br />PROVIDED WITH RENTAL ASSISTANCE AND WHETHER AND HOW THE AMOUNT OF THAT ASSISTANCE <br />WILL BE ADJUSTED OVER TIME. <br />Due to the limited funds and the desire to assist the greatest number of people possible, the <br />standard length of assistance will be three months with the goal to be self-sustaining <br />afterwards. Although situations may arise where additional assistance may be required.
