SUBSTANTIAL AMENDMENT TO THE I 2010/11- 2014/15 CITY OF SANTA ANA CONSOLIDATED PLAN <br />2011/12 ACTION PLAN <br />additional assistance will be provided. The city and sub recipients will continue their effort to <br />better describe circumstances that would allow for additional assistance. <br />STANDARDS FOR DETERMINING THE TYPE, AMOUNT, AND DURATION OF HOUSING STABILIZATION <br />AND/OR RELOCATION SERVICES TO PROVIDE A PROGRAM PARTICIPANT, INCLUDING THE LIMITS, IF <br />ANY, ON THE HOMELESSNESS PREVENTION OR RAPID RE-HOUSING ASSISTANCE THAT EACH <br />PROGRAM PARTICIPANT MAY RECEIVE, SUCH AS THE MAXIMUM AMOUNT OF ASSISTANCE, <br />MAXIMUM NUMBER OF MONTHS THE PROGRAM PARTICIPANTS RECEIVES ASSISTANCE; OR THE <br />MAXIMUM NUMBER OF TIMES THE PROGRAM PARTICIPANTS MAY RECEIVE ASSISTANCE. <br />Sub recipients will be allowed to provide the eligible ESG services in order to prevent <br />homelessness or move an individual or family into permanent housing. As previously indicated, <br />the standard amount of assistance will be based on the gap needed to be filled with ESG funds <br />and the standard length of assistance will be three months. <br />DESCRIBE PROCESS FOR MAKING SUB-AWARDS <br />The City makes its allocation available to non-profit organizations through the issuance of a <br />notice of funding availability. Any 501(c)(3) organization in good standing may submit a request <br />for funding for an eligible ESG activity. These applications are then reviewed and evaluated by <br />an ad hoc committee that makes recommendations for funding based on rating criteria. The <br />recommendations are forward to the City Council of Santa Ana for final approval, amendment <br />or rejection. <br />HOMELESS PARTICIPATION REQUIREMENT <br />The City will consult with the CoC in making policies and decisions regarding the use of ESG <br />funds. Through meeting and consulting with the CoC the City has determined that this <br />requirement will be met, since the CoC has a strategy that includes participation of all 34 cities <br />in Orange County, County Agencies, the County's homeless housing and service providers, and <br />other community groups, including non-profits, local governmental agencies, faith-based <br />organizations, the homeless and formerly homeless, interested business leaders, schools and <br />other stakeholders to identify the gaps and unmet needs of the County's homeless <br />PERFORMANCE STANDARDS <br />The City of Santa Ana will use the outcome standards indicated in the City's 5-year Consolidated <br />Plan as a preliminary performance standard.
