TRANSPORTATION ASSISTANCE - GENERAL GUIDELINES <br />Transportation assistance is defined as the provision of gas cards or a bus pass to <br />participants. <br />1. Need will be established by indicating the client is in need of supportive service <br />assistance through the following: <br />a. Adult/Dislocated Worker Clients - completion of the Participant <br />Informational Worksheet, budgeting form <br />b. Youth - completion of My Action Plan (MAP) <br />2. Case manager will complete an ITA form to indicate what supportive services will be <br />provided and will justify in the comment section of ITA. The amount may change <br />based on extenuating circumstances, at which point the ITA form may be revised <br />(youth case managers are exempt). <br />3. Transportation Assistance during Training-supportive services are available to clients <br />enrolled in WIA training services for up to 12 months, or the length of the training <br />program. Case Manager must verify participant attendance in WIA training services. <br />4. Transportation Assistance during Job Search - supportive services are available to <br />clients enrolled in WIA services during job search and can be delivered for up to 3 <br />months. Extensions may be provided on a case-by-case basis. <br />a. Qualifying criteria includes: <br />i. Participates in all required activities as agreed to by the case manager. <br />ii. Participant must provide evidence that he/she is looking for work on a <br />monthly basis. Participant must keep a job search log of outside <br />appointments/interviews to verify participant is actively seeking employment. <br />5. Transportation Assistance during Follow-up - supportive services are available to <br />youth for up to 12 months after Exit. <br />6. Gas Cards and/or Bus Passes <br />a. Case manager is responsible for the following: <br />i. Adult/Dislocated Worker Clients - completion of the Transportation <br />Supportive Services Statement form <br />ii. Youth - completion of Individual Supportive Service Form and Youth <br />Supportive Service Log. <br />b. Participants must request a gas card/bus pass in a timely manner. Gas card/bus <br />passes will not be issued on a retroactive basis. <br />Exhibit A 18
