REPORTING REQUIREMENTS <br />8.1 CONTRACTOR shall maintain records, collect data, and <br />provide reports mandated by Federal and State governments and as <br />may be required by ADMINISTRATOR. Reporting requirements shall <br />include all reports and data collection that is required to track <br />goals and report progress as noted in Paragraph 1 of this Exhibit <br />A to this Agreement. <br />8.2 Reports of problems, including attendance issues, <br />achievements, or other inquiries about VTR/WEX activities shall <br />be addressed immediately with the affected participant and <br />employer/instructor; CONTRACTOR shall inform WTW staff within <br />twenty-four (24) hours, through verbal or electronic <br />communication, which will allow for quick intervention and <br />results oriented action to address the issue with the <br />participant. <br />8.3 CONTRACTOR shall report each participant's monthly <br />attendance and progress, including achievements, to WTW staff by <br />the tenth (10th) calendar day of the following month in a report <br />format approved by ADMINISTRATOR. <br />8.4 CONTRACTOR shall provide, by the tenth (10th) calendar <br />day of each month, a status report for the preceding month, in a <br />format approved by ADMINISTRATOR. The monthly status report <br />shall identify, but not be limited to, the following: <br />8.4.1 Referrals received <br />8.4.2 Participants enrolled <br />8.4.3 Participants referred back to ADMINISTRATOR for <br />non-attendance <br />(WAM0313) 10 of 22 <br />(April 8, 2013) <br />25B-60
