Laboratory logbooks will be maintained to ensure timely collection and <br />reporting of results. Logbooks will also be used to evaluate the utilization <br />of services. <br />0. MEDICAL RECORDS <br />CMCMC will maintain all medical records in accordance with, CAC Title <br />15 guidelines, and all other applicable codes and regulations. A complete <br />medical record will be initiated for each inmate. <br />CMCMC requires accurate and timely reporting from all members of its <br />medical staff and enforces policies requiring thorough documentation of <br />all patient encounters (including mental health encounters). Each record <br />will include, but not be limited to, a problem list, progress notes, receiving <br />screening, health assessment forms, record of medication administration, <br />and diagnostic test reports. <br />Medical records will accompany the inmate to on-site health care <br />encounters and will be secured separately from confinement records. A <br />transfer summary form and copies of pertinent medical records will <br />accompany inmates transferred to other correctional facilities, as well as to <br />off-site medical appointments and/or hospital transfers. Copies of medical <br />records generated off-site will be requested for inclusion in the inmate's <br />medical file. <br />CMCMC acknowledges that all medical records are the property of the <br />City and will be maintained by CMCMC only while its contract is in <br />force. All records will be kept confidential, but will be accessible to <br />authorized medical personnel (including dental and mental health).
