OVERVIEW OF VERIFICATION REQUIREMENTS <br />H \[Notice 2010-9\] <br />HUD mandates the use of the EIV system and offers administrative guidance on the use of other <br />methods to verify family information and specifies the circumstances in which each method will <br />be used. In general HUD requires the PHA to use the most reliable form of verification that is <br />available and to document the reasons when the PHA uses a lesser form of verification. <br /> <br />SAHA Policy <br />In order of priority, the forms of verification that SAHA will use are: <br /> Upfront Income Verification (UIV) <br />Verification (EIV) system <br /> Upfront Income Verification (UIV) <br />using non-HUD system <br /> Written third Party Verification <br />(may be provided by applicant or <br />participant)dispute) <br /> Written Third Party Verification Form <br /> <br /> Oral Third Party Verification <br /> <br /> Self-Certification <br /> (Use as a last resort when unable to obtain any type of third <br />party verification) <br /> <br />Each of the verification methods is discussed in subsequent sections below. <br />Requirements for Acceptable Documents <br /> SAHA Policy <br />Any documents used for verification must be the original (not photocopies) and generally <br />must be dated within 60 calendar days of the date they are provided to SAHA. The <br />documents must not be damaged, altered or in any way illegible. <br /> Print-outs from Web pages maybe considered original documents <br />The SAHA staff member will review the original document against the photocopy, <br />indicate date viewed and initial. <br />SAHA may accept documents more than 60 days before the effective date of the family's <br />reexamination if the document represents the most recent scheduled report from a source. <br />For example, if the holder of a pension annuity provides semi-annual reports, SAHA <br />would accept the most recent report. <br /> <br />Any family self-certifications must be made in a format acceptable to SAHA. <br />File Documentation <br />The PHA must document in the file how the figures used in income and rent calculations were <br />determined. All verification attempts, information obtained, and decisions reached during the <br />il to demonstrate that <br />SAHA has followed all of the verification policies set forth in this plan.
