The SSE will evaluate trainees on a scale of 1 to 5 on each of the Steps to Success categories. The evaluation must be <br />completed in ETO and a rubric has been established to guide staff with the evaluation. The SSE rubric can be found in <br />Appendix B of this manual and in Staff docs /Support Services /Case Management /Assessments. <br />Instructions for entering SSE into ETO follow: <br />1. Go to the home page for your program (Medical Careers, Office Careers, Taller Tech) <br />2. In the search bar above, enter the name of applicant (Quick Search) <br />3. Once you see the name, click the arrow next to the name, and then click "View /Record Assessment" <br />4. Click "Take New Assessment" <br />5. Select "Steps to Success Evaluation" and "Continue" <br />6. Fill in appropriate scores for each area <br />Individual Services Plan (ISP) <br />The Individual Service Plan (ISP) is a road map developed by the Support Services Specialist in conjunction with the <br />participant. It aims to help the participant reach their goals and assist staff in meeting Taller San Jose's outcomes. <br />When to Create and Update the ISP: <br />The ISP must be completed and updated at these points: <br />✓ Orientation. To be completed for all participants by end of week 1 of training. <br />✓ End of training. Complete during "Next Steps" meeting and no sooner than 3 weeks before training <br />completion. During the "next steps meeting ", the Support Services Specialists should review the results from <br />the post -TABE test and the Casey Life Skills Assessment and set goals for 90 days, 1 year, 18 month and 2 year <br />after training completion. Staff should also use the time to give feedback to the trainee about their strengths <br />and opportunities for improvement. Education and employment are the priority areas on the ISP at this <br />juncture. <br />✓ 30, 60, 90 days after training completion. Goals may not necessarily be updated at these intervals however <br />all other questions on the ISP must be completed. <br />✓ 1 year after training completion <br />✓ 18 months after training completion <br />✓ 2 years after training completion <br />A sample of the ISP can be found in in Staff docs /Support Services /Case Management /Assessments. <br />Instructions for entering ISP into ETO: <br />1. Log onto ETO using your username and password <br />2. Go to Home page for your program (Medical Careers, Office Careers, Taller Tech) <br />3. In the search bar above, enter the name of applicant (Quick Search) <br />4. Once you see the name, click the arrow next to the name, and then click "View /Record Assessment" <br />5. Click "Take New Assessment" <br />6. Select "ISP" and "Continue" <br />7.
