✓ Focuses on an individual's strengths <br />Placement Phase 1.:1 Meetings <br />Support Services Specialists are responsible for continuing to provide comprehensive support services during placement. <br />This is facilitated through one -on -one meetings with interns. Meetings must be provided accordingly and be documented in <br />Taller San Jose's data management system. Refer to Appendix D for a visual of the content below: <br />Training Completion -90 days: Weekly meetings for Interns with low to mid scores on Casey, SSE and /or those <br />without jobs at training completion or when a concern exists that the Intern will <br />have difficulty obtaining and retaining a job due to social and emotional issues <br />or criminal background. Approximately 80% of youth who advance to Phase 4 <br />will fall into this category. <br />Length of Meeting: At least 30 minutes per Intern <br />Training Completion — 90 days: Meetings with Interns who score high on Casey, SSE and /or demonstrating <br />greater stability should take place at end of 30, 60, 90 days after training <br />completion <br />Length of Meeting: 45 minutes maximum for all three meetings <br />90 days —1 Year: Monthly meetings for Interns with low to mid scores on Casey, and /or greater <br />instability based on assessment <br />Length of Meeting: At least 30 minutes per Intern <br />90 days — 1 Year: One meeting every 4 months after training completion for high functioning <br />group <br />Length of Meeting: Approximately 30 minutes per Intern <br />1 Year— 2 Years: Meetings during data collection periods 1 Year, 18 month, 2 Year and every 3 <br />months for all Interns <br />Length of Meeting: Approximately 30 minutes per Intern. During data collection periods, expect to <br />spend closer to 45 minutes. <br />Characteristics of an Effective 1:1 Support Meeting during Placement Phase <br />An effective meeting has the following characteristics: <br />✓ Drives Interns to accomplish ISP goals <br />✓ Revises goals when needed <br />✓ There is a constant assessment of Intern's needs and barriers <br />✓ Links to appropriate services external and internal (i.e. social and emotional wellness group, educational <br />groups, job clubs, therapy, etc...) <br />✓ Addresses any issues that need immediate attention <br />Page 37 Taller San Jose, Support Services Manual EXHIBIT A <br />
