There will be one full -time lead Case Manager assigned to oversee this program. There will be <br />one Assistant Case Manager for Out —of— School Youth. Case managers will be assisted by part- <br />time Clerk, Sr. Tutor and Tutor staff assigned to the project. Students have the opportunity of <br />interacting with their respective Case Manager at each class, work experience or workshop. On <br />average students and Case Managers will interact 4 -5 times a week. For more intensive needs of <br />the students, the Case Manager and/ or additional support staff will be available. <br />3. Population <br />A. Describe the total number of youth that will be served by this grant. <br />The Seeds to Trees Academy program will serve 22 youth. <br />B. Describe the age range of the youth that will be served. <br />Of the 22 youth, 15 will be "out -of- school youth" (ages 16 -24) and 7 will be "in- school <br />youth" (ages 14 -21). <br />C. Describe any minimum requirements that youth need to have in order to be enrolled <br />into your program (aside from the W10A Youth eligibility requirements). <br />In addition to WIOA Youth eligibility requirements, priority enrollment will be provided to <br />WIOA eligible returning military veterans who are out-of-school youth (refer to letter of support <br />from SAC Veteran Upward Bound program). Additional priority enrollment will be provided to <br />WIOA eligible youth who have completed a minimum of 30 hours of volunteer service in Santa <br />Ana Public Library and /or Parks and Recreation programs. <br />D. Special consideration for organizations that can serve the following youth populations: <br />1. At least 75% of Out -of- School youth (required): Yes we will serve 75% this population <br />2. At least 10% of youth with disabilities: Yes 10% will be this population <br />3. At least 10% of foster /emancipated youth: Yes 10% will be this population <br />4. At least 10% youth that have dropped out of high school: Yes 10% will be this population <br />5. At least 10% of youth on probation: Yes 10% will be this population <br />E. Specify how the targeted youth populations (out -of- school, youth with disabilities, <br />foster /emancipated, probation, dropouts) will be recruited. <br />• Out -of- School and dropout youth will be recruited via referrals from SAC Continuing <br />Education Program, SAUSD (refer C2),Access California services (refer to letter of <br />intent E9) and at outreach events. We will also post flyers and recruit for the program <br />among library patrons (refer to attachment W4). <br />• Youth with Disabilities: The library has had a successful association with serving as a <br />work experience site for the Santa Ana Unified School District's Special Education <br />Program Youth. The Transition Center has agreed to continue to refer youth with <br />disabilities to the Seeds to Trees Academy (please refer to Letter C4 from SAUSD <br />Transition Center). Youth with physical disabilities will be welcome in the program, as <br />will youth with mild learning disorders. We have had tremendous success working with <br />youth who are autistic, and have started a partnership with Easter Seals Autistic Services <br />program. They have agreed to refer youth to our program as well (please refer to letter <br />C5 from Easter Seals Autistic services). Youth with severe cognitive disabilities will find <br />EXHIBIT A <br />
