Statement Regarding Apprenticeship Requirements <br />13, 5taternentRegardIng "Anti-Kickback"Requirements <br />14, Public Contract Cade Section 10152 Questionnaire <br />The Proposal may be considered non responsive if any of these documents or forms is not <br />Included, The bid package shall be submitted as instructed in the Notice Inviting Bids, it is the <br />BIDDER'S responsibllity, to ensure delivery of the Proposal to the location and prior to the bid <br />opening hour stipulated in the Notice Inviting Bids, Late Proposals will not be considered. <br />CALIFORNIA STATE LABOR CODE <br />As outlined In the Notice inviting Bids, this project is subject to compliance monitoring and <br />enforcement by the State of California Department of Industrial Relations, per Section <br />1771.4.a.1. BIDDERS are required to inform themselves fully of the conditions relating to <br />construction and labor under which the work will be performed, Any contract entered Into <br />pursuant to this notice will incorporate the provisions of the California State Labor Code.
