NaphCare will ensure policy and procedures adhere to guidelines established by the National <br />Commission on Correctional Health Care (NCCHC), Orange County Health Authority and Title 15 <br />and 24 regulations for adult facilities. <br />PROACTIVE CHRONIC CARE MANAGEMENT <br />NaphCare provides chronic care in a manner that <br />incorporates principles of case and disease management <br />for complex cases, and promotes maximum progress <br />and healing. Inmates receive timely follow-up, <br />evaluation, treatment, and education about the <br />preventive activities available for those requiring chronic <br />or convalescent care. Our policy ensures all inmates are <br />screened, identified, and monitored in a manner <br />consistent with national clinical guidelines established <br />for the care and treatment of chronic illnesses. <br />Our proactive approach and <br />effective management of <br />inmates with chronic diseases <br />enables healthcare professionals <br />to treat symptoms earlier, and <br />more effectively, preventing <br />costly hospitalizations. <br />We take a proactive approach to the management of chronic care disease in order to minimize the <br />development of any urgent or emergent conditions that might require off -site transportation. Our <br />emphasis on preventive care begins at the point of intake (intake screening), where inmates are <br />classified into the appropriate chronic care clinic and scheduled forfollow-up treatment. Our <br />extensive staff training, use of best practices based on nationally recognized guidelines, and <br />innovative on -site diagnostic testing help us keep chronic care patients in a stable state throughout <br />their incarceration. <br />We schedule and track all chronic care clinic visits within TechCare®. This data will be available to <br />the city at all times. At a minimum, the database will include the following: <br />• Each inmate enrolled in a chronic care clinic. <br />• Each occasion when an enrolled inmate is seen at a chronic care clinic. <br />• Patient refusals for a chronic care visit. <br />Using Tech Care® helps ensure that chronic care patients are seen by a provider at appropriate intervals <br />as clinically indicated. In the Chronic Disease Management section of TechCaree, the user can <br />access/view inmates with certain chronic care illnesses. To see a specific chronic illness, simply select a <br />chronic illness from the drop down list. The user can view previous notes, labs, and chronic care visits, or <br />complete a chronic care visit. The amount of time until the next visit can also be reviewed from this <br />screen.
