Using TechCoreO, we strive to enhance the level and quality of the care within the <br />SAJ by scheduling inmates with chronic illnesses to be seen by a provider before they become acutely ill <br />and require off -site transport or hospitalization. In addition, we greatly reduce liability by identifying <br />this high -risk population during the intake process. <br />Collection of Pertinent Healthcare Information <br />Prior to incarceration, many inmates had limited contact with healthcare providers and may lack critical <br />information about their illnesses. Our chronic care program aims to actively monitor, educate, and <br />motivate patients to be responsible for their own health maintenance. We have established protocols <br />and practice guidelines to provide guidance on the diagnosis, monitoring, and treatment of common <br />chronic illnesses. Our process ensures compliance with standards established for the care and <br />treatment of chronic illnesses. <br />The first opportunity to identify, enroll, and refer an inmate to an advanced level provider is during the <br />Intake Screening. If the inmate's responses during intake indicate that he or she requires additional <br />medical care, then the inmate's medical record is electronically flagged for follow-up and, typically, their <br />chronic issues are addressed by the provider during that initial health assessment. If a patient is on <br />pharmacologic therapy, continuity will be maintained. If there is a patient whose chronic condition is <br />unstable, he/she will be seen promptly. In the case where a patients' chronic disease is stable, he/she <br />will be scheduled for a first chronic disease visit in approximately one month. NaphCare is flexible and <br />will work on custody -related issues to reduce interruption of chronic care medications or appointments <br />when patients with chronic disease are transferred between institutions or moved for housing, court or <br />release issues. <br />Before the appointment, we collect medical records and current diagnostic test results so an evidence - <br />based treatment plan may be created. Patients enrolled in chronic care clinics will be seen by a <br />qualified healthcare professional at appropriate intervals, or more frequently if clinically indicated. <br />Inmate Medical Services RFP No. 17-051 <br />City of Santa Ana 36 <br />
