Seman Date of Report: 01/22/2013 <br /> <br />Page 1 of 2 <br /> <br /> <br />CASE NAME: GUILLEN, JAIME LOPEZ <br />CLASSIFICATION: OFFICER INVOLVED SHOOTING (NON-FATAL) <br />AGENCY: SANTA ANA POLICE DEPARTMENT <br />DATE AND TIME: OCTOBER 22, 2012 2209 HOURS <br />LOCATION: 210 SOUTH BIRCH STREET, SANTA ANA, CA <br />AGENCY CASE NUMBERS: SAPD: 12-31262 <br />OC CRIME LAB: 12-55780 <br />On Tuesday, October 22, 2012, at approximately 2222 hours, I spoke with Santa Ana <br />Police Commander STEVE COLON who advised of an officer involved shooting, which occurred <br />at 520 West Walnut Street in the city of Santa Ana. COLON requested the Orange County <br />District Attorney’s Officer Involved Incident Investigation Team, respond to the location and <br />conduct an investigation into the incident. <br />I notified District Attorney Investigators PAUL KELLY, PAUL CARVO, RICK BRADLEY, TOM <br />CONKLIN, ANDREW ROSS, BRETT ANDERSEN, FRANK CARDENAS and FRANK REYNOSO and <br />requested they respond to assist in the investigation. I went to the shooting location and met <br />with personnel assigned to the incident. <br />On October 23, 2012, at approximately 0037 hours, Santa Ana Police Sergeant JIM <br />ARMSTRONG conducted a briefing on the known circumstances surrounding the incident for <br />personnel involved in the investigation.
