w` REQUEST FOR PROPOSAL <br />uu, Water Resources Construction Management and Inspection Services <br />lw (RFP NO. 19-118) <br />Sewer Cement Pipe Rehabilitation 1 Replacement Group 13 & <br />15 <br />Long Beach, CA <br />As part of the Michael Baker on -call services agreement, we were <br />selected by the LBWD to provide construction inspection services <br />for this pipeline relocation project necessitated by the construction of <br />the New Gerald Desmond Bridge in Long Beach. The work involved <br />approximately 200 If of 16" DIP and 1,900 If of 24" DIP waterline. <br />Michael Baker's duties encompassed: providing construction inspection <br />and contract administration; <br />verifying quantities and insuring aet <br />quality control; and preparing daily <br />construction reports, digital photos' <br />and the punch list. <br />Relevance to the City: <br />• Active site at the Port of <br />Long Beach <br />• Coordination with <br />Gerald Desmond <br />Bridge Contractor <br />Team Members Involved: <br />Kieler Smith, PE <br />Jerome Ruddins, CCM <br />Eric Eljenholm, EIT <br />Nora Doyle <br />Client: Long Beach Water <br />Department <br />Dates: 3/2016 - 11/2016 <br />Contact: <br />Robert Verceles, PE <br />Phone: <br />(562)570-2337 <br />North Long Beach Sewer Improvement Project Phase 2 <br />Long Beach, CA <br />Michael Baker was selected by the LBWD to provide construction <br />management and inspection services for this sewer improvement <br />project located in —24 different areas within the City of Long Beach. <br />Work included: the rehabilitation of—10,013 LF of sewer pipe using <br />CIPP lining, pre- and post -CCTV inspections, cleaning and lining 8" and <br />10" pipe, point repairs, removing and replacing sections of existing 8" <br />and 10" sewers, restoring existing sewer laterals after repair/lining, root <br />ball removal, installation of top hat repairs, repair of intruding laterals, <br />repair manhole channels, trim joint offset, temporary sewer bypass <br />facilities, sewage spill prevention plan, post installation pipeline testing, <br />traffic control, and potholing. Michael Baker's duties included: providing <br />construction management, inspection, and contract administration; <br />verifying quantities and ensuring quality control; providing public <br />relations and monitoring the Contractor's safety plan; coordinating <br />survey and material testing; scheduling and coordinating temporary <br />shut -downs; conducting meetings and preparing minutes; monitoring <br />project schedules and permits; and preparing and processing control <br />documents such as RFI's, submittals, progress payments, change <br />orders, daily construction reports, digital photos, weekly statements of <br />calendar days, and the final punch list.
