a) Provide screening and needs assessments to identify danger, medical needs, legal <br />and law enforcement needs. <br />b) Provide emergency assistance to meet physical needs, such as food, hygiene, <br />clothing, and transportation. <br />c) Provide free -of -cost in -person and telehealth behavioral health counseling <br />services. <br />d) Provide one-on-one case management sessions to provide direct service referrals <br />and resource linkages. <br />e) Provide independent living skills through case management, counseling, and <br />support groups. Services are focused on assisting participants to develop critical <br />life skills and coping skills that are essential functions to regaining their <br />confidence, safety, and self-sufficiency. <br />f) Provide support and education to participants regarding victim rights, legal <br />systems and processes, medical processes, and law enforcement functions. <br />g) Refer participants in need of additional community support services and resources <br />to the City of Santa Ana and to other participating organizations in the CDBG <br />cohort. <br />4. DURATION: This MOU shall commence on September 1, 2024, and shall remain in effect <br />through June 30, 2025, with the option for the City to grant up to two 1-year renewals, <br />exercisable by a writing by the City Manager and City Attorney, unless terminated earlier in <br />accordance with Section 10 below. <br />5. AMENDMENTS: Either Party may propose amendments to this MOU at any time by <br />providing written notice to the other Party. However, this MOU may not be modified except <br />by written instrument signed by the City and authorized representative of Subrecipient. . <br />6. CONFIDENTIALITY: If Subrecipient receives information, which due to the nature of <br />such information is reasonably understood to be confidential and/or proprietary, such <br />information shall not be used or disclosed except in the performance of this MOU, and <br />Subrecipient agrees to exercise the same degree of care it uses to protect its own information <br />of like importance, but in no event less than reasonable care. "Confidential Information" <br />shall include all non-public information. Confidential information includes not only written <br />information, but also information transferred orally, visually, electronically, or by other <br />means. Confidential information disclosed to either Party by any subsidiary and/or agent of <br />the other Party is covered by this MOU. The foregoing obligations of non-use and <br />nondisclosure shall not apply to any information that (a) has been disclosed in publicly <br />available sources; (b) is, through no fault of Subrecipient disclosed in a publicly available <br />source; (c) is in rightful possession of Subrecipient without an obligation of confidentiality; <br />(d) is required to be disclosed by operation of law; or (e) is independently developed by <br />Subrecipient without reference to information disclosed by the City of Santa Ana. <br />7. ACCESSABILITY: Subrecipient will assure that its services and premises are accessible to <br />persons with disabilities pursuant to the requirements of the Americans with Disabilities Act. <br />
