VII. ESTABLISHING OF FEES <br />The Consultant shall furnish a fee schedule for the proposed services. The fee schedule shall depict the <br />hourly rates for each personnel category to be used on the project. Personnel hourly rates shall reflect <br />all costs for office overhead, including phones, cellular phones, vehicles, mileage and other direct and <br />indirect costs. Also the fee proposal shall reflect all anticipated fee increases during contract duration. <br />The Consultant shall also complete Attachment "A" (Consultant Rate Comparison Form). The <br />attachment is located at the end of the Request for Proposal and must be included with the <br />comprehensive fee schedule in a separate sealed envelope. <br />The contract will be issued for the total not to exceed amount of $50,000. A detailed scope of work <br />will be outlined when specific project is assigned to the Consultant and then the fee for each assigned <br />project will be negotiated. <br />VIII. INSURANCE REQUIREMENTS <br />A certificate of insurance is enclosed for your reference as Attachment "B". Please refer to the sample <br />agreement for the necessary amounts of general liability, automotive, worker's compensation and <br />professional liability insurance. The appropriate endorsements are also shown within the sample <br />contract agreements. The certificate shall include the City and its officers and employees as insured or <br />additional insured. <br />IX. PROFESSIONAL SERVICES AGREEMENT <br />A.sample Professional Services Agreement is enclosed for your review, as Attachment "C". The RFP <br />and the Consultant's proposal will be attached and become part of the agreement as exhibits. <br />XI. COORDINATION <br />I. Coordination with the City, other consultants and other involved agencies will be <br />required to achieve satisfactory and timely delivery of the final work product. <br />Coordination may include, but not be limited to, coordination with Caltrans, FHWA, <br />Environmental Protection Agency, State Water Quality Board, County of Orange, and <br />other agencies and consultants. <br />2. The City will decide the manner in which the coordination of individual matters is <br />undertaken. At the City's option, coordination efforts may be performed by the <br />Consultant's direct contact, by the Consultant acting through the City or by the City <br />only. When coordination efforts require agreements, such agreements shall be <br />coordinated through the City. <br />rel <br />
