<br />Application for Training and Educational Assistance <br />form and submit it to the Police Chief. <br /> <br />B. The Police Chief will recommend approval or disapproval <br />and forward the application to the Personnel Services <br />Department. It is advisable that the applicant <br />accomplish the procedures so far described prior to the <br />inception of the course or disbursement of personal <br />funds in order to ascertain the eligibility of the <br />intended course of instruction for reimbursement under <br />the provisions of this policy. <br /> <br />C. Wi thin three months after he/she has completed the <br />course and received his/her final grade, the employee <br />must include official verification of his/her final <br />grade with appropriate receipts for tuition and <br />textbook costs to the Personnel Services Department. <br />These will be returned to the employee upon request. <br />Applications not submitted to the Personnel Services <br />Department within three months following completion of <br />the course become void. <br /> <br />D. Upon receipt of the required documentation, the <br />Personnel Services Department will determine whether <br />the completed course of instruction is compatible with <br />the provisions of Sections 2 and 3 of this Article. If <br />found to be compatible, the Personnel Services <br />Department will compute the amount of reimbursement, <br />authenticate the application and forward it to the <br />Police Chief. <br /> <br />E. <br /> <br />The Police Chief will <br />Management Services <br />employee the approved <br />Police Department. <br /> <br />then authorize the Finance <br />Department to reimburse <br />amount out of the budget of <br /> <br />and <br />the <br />the <br /> <br />29 <br />
