<br />AH-cH~ ",e"r I <br /> <br />Change Order #1 - Revision a <br />Page 2 <br /> <br />9: Building Dept. required new office: +52,705.00 <br />Provide a new 10')(.12' office on the main floor to reduce the open area square footage to allow <br />the intended use meet the maximum allowable square footage for open area without having to <br />install new fire sprinklers. <br /> <br />10: Building Dept. required fees. +51,473.75 <br />Add for the cost of accelerated plan check fees and the re-roof permit fee llS charged by the city. <br /> <br />11: Fire Dept. required building address numbers: <br />Provide new 6" building wall mounted address numbers as required. <br /> <br />+$118.00 <br /> <br />12: Add for mold testing: +$295.00 <br />Due to the hellVY winter rains and that the roofwas leaking badly, the building was contaminated <br />with a heavy mold smell. For safety reasons, we hired a certified mold inspection company to <br />test for the presence of mold. A report was forwarded to the city. <br /> <br />13: Add for mold removal: +$3,598.00 <br />The mold testing report reveled heavy concentrations of dangerous mold within the building. <br />The mold detection company provided a proposal to remove the mold in the building. The cost <br />for that removal was $18.390.00. A copy of that proposal was forwarded to the city. We felt that <br />the proposal remove the mold went further than was required. We havc removed tbe mold <br />affected materials in house, which provided a valuable savings to the city. This proces>; included <br />removing all the water damaged floor tiles and ceiling tiles and the ceiling insulation. We also <br />removed the water damaged ventilation ducts, damaged drywall, lIlld chemically cleaned all the <br />existing walls, floors and remaining areas. <br /> <br />14: Add for dry-rot roof plywood: +55,730.40 <br />Add for replacing (76) sheets of roof plywood damage by the leaking roof membrane. <br /> <br />15: Add for removing multiple layers roof membranes: +S800.oo <br />Add for removing extra layers of roofing membrdlles that were under the top layers. Our original <br />bid did not include removing extra layers of roofing membranes. We were not able to detect the <br />extra layers until we started removing the roofing materillls. <br /> <br />16: Add for replacing dry-rot structural framing: +$5,422.00. <br />After the existing roofing was removed, we found severe structural roof framing damage to the <br />buildings wood truss system. After informing our city contact, we called our structural engineer <br />for direction and then proceeded tn replace all the damaged areas. <br />
