CONTRACTUAL OBLIGATIONS: The City shall require certain contractual obligations. <br />These may include but are not limited to the following: <br />Inclusion of Proposal: The proposal submitted in response to this RFP will be required to be <br />included as part of the final contract with the selected proposer. <br />Inclusion of RFP: This RFP will be required to be included as part of the final contract with the <br />selected proposer. <br />Insurance: The successful proposer shall not start work until all insurance requirements meet <br />the approval of the City. The proposer is also responsible for all subcontractor insurance <br />requirements. <br />Certificates of insurance shall be in duplicate and addressed to the City of Santa Ana. All <br />insurance shall remain in effect during the life of the contract. Certificates must indicate that the <br />City shall receive written notice thirty (30} days prior to any cancellation or reduction in <br />coverage. <br />1. Comprehensive General Liability insurance to include Completed Operations, Contractual, <br />Independent Contractors and Personal Injury; and Automobile Liability, including all auto; <br />with at Least the following combined Limits of Liability: <br />(a) Primary Bodily Injury Liability limits of $1,000,000.00 per occurance <br />(b) Primary Properly Damage Liability limits of $1,000,000.00 per occurance <br />(c} Primary Bodily Injury and Property Damage Liability combined single limit of <br />$1,000,000.00 is preferred <br />2. Medical malpractice insurance of not Less than three (3) million dollars per occurrence. <br />3. Employer's Liability with a combined single limit of $1,000,000.00 <br />4. Worker's compensation insurance, with limits established and required by the State of <br />California. <br />4 <br />
