5. No more than twenty (20) days after notification of award, a separate additional insured <br />endorsement is required for part "1" above (Commercial general liability insurance) <br />naming the City of Santa Ana, its officers, agents, employees and volunteers as additional <br />insured. <br />Selection: The final award of a contract is subject to approval by the Santa Ana City Council. <br />Contract: A resulting contract may include any written material made as part of your proposal. <br />This material may include but is not limited to; answers to questions contained in this RFP <br />letters, telegrams, facsimile and product literature. <br />STATUTES AND RULES: The terms and conditions of the RFP, the resulting services, and <br />furnishing of commodities based on the RFP shall conform to the statutes, rules and regulation of <br />the Federal Government, State of California, City of Santa Ana and the Santa Ana Police <br />Department. <br />PROPOSAL INCLUSIONS: Five (5) copies of the proposals, all information requested in this <br />RFP and the following items must be submitted in writing: <br />• A declaration of the proposers ability to perform the services requested and meet all <br />qualifications, terms and conditions outlined. <br />• A description of the vendor's correctional medical experience. <br />• A reference list that includes all of the proposers correctional clients, located in the State of <br />California, for the previous three (3) years, whether they are currently clients or not. <br />References should include name, location and size of each facility and name and phone <br />number of a knowledgeable contact person for each. <br />• Proposals must clearly indicate the legal name, address and telephone number of the <br />proposer. Proposals shall be signed above the typed or printed name and title of the <br />signatory. The signatory shall have the authority to bind the proposer to the submitted <br />proposal. <br />• Any changes or modifications to the terms and conditions the proposer wishes the City to <br />consider. Proposers are encouraged to offer any other options that may be of interest to the <br />City, with the understanding that their proposal must first address the RFP requirements. <br />Options are to be identified as such. <br />5 <br />
