Clinical facilities requ re <br /> that a fiealth screening check must be completed prior to the start of the clinical learni g <br /> experience. Thus, the CNA student is required to complete the health screening <br /> requirements before the start of the program. Submission of requirements alone does not <br /> indicat completion - the health screening form must be validated and approved by th <br /> Prograin Director. <br /> A student who fails to complete the health screening requirement will not be allowed Lo <br /> attend linical rotation. Such will result in clinical absences and may cause dismissal <br /> from tf e program. <br /> The stL dent shall cover the cost of health screening. <br /> Health screening consists of physical examination, TB screening test, antibody titers, ind <br /> immun izations. <br /> PHYSICAL EXAMINATION <br /> The stt dent must be certified by his/her primary health provider that he/she is physice lly <br /> and psychologically fit to perform the functions of a student vocational nurse. A student <br /> who is not certified fit must submit additional certification that lists restrictions. A <br /> physic•lly and/or psychologically unfit student shall be deferred of enrollment or <br /> prograin admission. Failure of applicant to disclose physical and psychological <br /> limitations releases Pacific College of any liability. <br /> PREG14ANCY <br /> All chi d-bearing applicants must be certified that they are not currently pregnant. If the <br /> app[ica nt is pregnant, she shall be required to submit a medical certificate that indicates <br /> she is E llowed to perform the duties and responsibilities of a student vocational nurse. <br /> The ap licant must also be counseled of the risks related to the program and shall be <br /> made t <br /> pregnancy restrictions shall be deferred of enrollment or program admission.
