The student will receive a copy of <br /> the End-of-Term Clinical Evaluation. <br /> Course and Faculty <br /> Students are responsible for evaluating CNA courses and faculty using electronic forms <br /> e-mat , , 'o, �,,.�,� d as 0%,� �-�� a� �.h <br /> analy " . Results are shared with the faculty member and the nursing administratio . The <br /> purpose of this process is to provide feedback on areas that are working well and ar as <br /> for in iprovement. <br /> Professional Guidelines <br /> Dress ode <br /> Stude its are required to buy two sets of navy scrub uniforms with LGL College log <br /> from GL College which the student must wear while in the clinical rotation, and/ol as <br /> requii ed by the CNA faculty. <br /> Students are to conform to the Dress Code Policy when on campus or in a clinical facility. <br /> Below is a list of guidelines a student must follow: <br /> 1. Personal hygiene is required of all students. The student must be neat, <br /> clean, and odor-free all the time. The instructor has the free-hand <br /> determination as what is acceptable. <br /> 2. A student has to maintain the appearance of their uniforms, The navy scrub t p <br /> and pants must be clean, wrinkle-free, and stain-free. They must not be ripped <br /> ortorn. <br /> 3. Black shoes with or without laces must accompany the scrub tops and pants. <br /> They should be kept clean at all times. Shoes must meet hospital safety <br /> standards.No open toe or open heel shoes are permitted. Socks must also be <br /> worn at all times. <br /> 4. Name badges must be worn at all times. The name of the student must be vis ble <br /> at all times, The student must not use school identification or uniform at work or <br /> in any activity not approved by the school. <br /> 5. A plain white sweater or jacket (without a hood) may be worn with the uniform. <br /> Under shirts may also be worn but must be plain white. Printed jackets and <br /> undershirts are not allowed. <br /> 6. Hair must be neat, clean, and worn above the collar. Long ponytails or hair tf at <br /> may fall forward are not acceptable. Hair accessories must be plain and smai . <br /> Hair color must be neutral. <br /> 7. Nails must be clean. No polish or artificial nails is allowed. Length of nails <br /> must not exceed inch. Nails must e a neutral color and without a design <br /> or accessories. <br /> 8. Students are required to have equipment such as a watch with a mechanism that <br /> records seconds, scissors, stethoscope with bell and diaphragm, penlight, <br /> protective eye wear as appropriate, and two black ink pens.
