BUILDING A BETTER EARLY CARE AND EDUCATION SYSTEM <br />(BBECES) (WIC 10420-10429.5; CIVIL CODE 1798.17; 42 US <br />9858C(2)(D) AND (U); 45 CFR 98.16(AA), 98.33 AND 98.42) <br />(Applies to all contract types that provide care through family childcare homes <br />and/or through individual licensed-exempt providers) <br />A. Submission and Disclosure of Childcare Provider Information <br />1. Contractors are required to collect and submit to the CDSS, or its designee, <br />as required by law, the following information for all licensed family childcare <br />home providers and individual licensed-exempt childcare providers providing <br />subsidized childcare services and in accordance with Section IV, Paragraph <br />J, Subsidized Provider Report: <br />a. Mailing address <br />b. Home address <br />c. County <br />d. Home, cell, and work telephone number, if known <br />e. Email address, if known <br />f. Whether provider is licensed or license-exempt, include state facility <br />license number for licensed providers. <br />g. Primary language used, if known* <br />h. Contractor, subcontractor, agency, or political subdivision administering <br />the program in which the provider participates. <br />i. Unique provider identification number, when available* <br />j. Contract type <br />k. The amounts of all subsidies paid to each provider in the report month, <br />Note: Provider work telephones number, email address, primary language used, <br />and unique provider identification number are not optional; if it is known to the <br />contractor, it must be included. <br />2. Contractors that are not able to provide amounts of all subsidies, as specified <br />in (k) of (1) above, shall have an implementation plan on file with CCPU . <br />3. The information collected from family childcare providers, as defined, may be <br />re-disclosed by the CDSS to provider organizations as defined in law as well
