B. With respect to employees who do have dependents <br /> included under the program, an amount equal to 90% of the total <br /> premiums applicable to the "family plan" covering said employees <br /> and their dependents. <br /> C. Notwithstanding the foregoing, with respect to <br /> employees hired after the effective date of this Agreement , the <br /> City will contribute toward the payment of premiums for health <br /> insurance on the following basis: <br /> (1) During the first year of employment, 100% of the pre- <br /> mium for "employee-only" coverage but no amount for dependent <br /> coverage.. The ',employee shall have the option to purchase depen- <br /> dent coverage under the Plans provided hereunder by personally <br /> paying the amount of difference between "employees-only" coverage <br /> and "family" coverage. <br /> ( 2) During the second year of employment, 100% of the pre- <br /> mium for "employee-only" coverage and 50% of the "additional <br /> amount" necessary to purchase "family" coverage, provided the <br /> employee purchases such family coverage. The employee shall be <br /> responsible for paying the remaining 50% of such "additional <br /> amount" . <br /> ( 3) After the completion of two years of service, the City <br /> shall contribute toward the payment of premium for health insur- <br /> ance in the same manner as provided for in A and B, above. <br /> Section 2. Dental Insurance. The City agrees to contribute <br /> the following amounts toward the payment of premiums for dental <br /> insurance plans provided by the City for employees covered by <br /> this Agreement and their eligible dependents : <br /> A. With respect to sworn, "safety" police personnel , <br /> up to a maximum of fifteen dollars and thirty-two cents ($15 . 32 ) <br /> per month per employee. <br /> B. With respect to "miscellaneous" employees, up to a <br /> maximum of twelve dollars ($12. 00 ) per month per employee. <br /> C. Any contribution necessary to maintain benefits <br /> under said dental plans in excess of the amounts set forth above <br /> shall be borne entirely by the employee. <br /> Section 3 . Long-Term Disability (L.T.D. ) Insurance. The <br /> City shall continue to contribute the following amounts toward <br /> the payment of premiums for employees covered by this agreement <br /> under the existing long-term disability plan maintained by the <br /> City for its employees; <br /> A. With respect to sworn, "safety" police personnel , <br /> up to a maximum of six dollars ($6 . 00 ) per month per employee; <br /> -28- <br />
