Insurance: The successful proposer shall not start work until all insurance requirements meet the approval of <br />the City. The proposer is also responsible for all subcontractor insurance requirements. <br />Certificates of insurance shall be in duplicate and addressed to the City of Santa Ana. All insurance shall <br />remain in effect during the life of the contract. Certificates must indicate that the City shall receive written <br />notice thirty (30) days prior to any cancellation or reduction in coverage. <br />1. Comprehensive General Liability insurance to include Completed Operations, Contractual, Independent <br />Contractors and Personal Injury; and Automobile Liability, including all auto; with at least the following <br />combined limits of Liability: <br />(a) Primary Bodily Injury Liability limits of $1,000,000.00 per occurance <br />(b) Primary Property Damage Liability limits of $1,000,000.00 per occurance <br />(c) Primary Bodily Injury and Property Damage Liability combined single limit of $1,000,000.00 is <br />preferred <br />2. Medical malpractice insurance of not less than three (3) million dollars per occurrence. <br />3. Employer's Liability with a combined single limit of $1,000,000.00 <br />4. Worker's compensation insurance, with limits established and required by the State of California. <br />5. No more than twenty (20) days after notification of award, a separate additional insured endorsement is <br />required for part "1" above (Commercial general liability insurance) naming the City of Santa Ana, its <br />officers, agents, employees and volunteers as additional insured. <br />Selection: The final award of a contract is subject to approval by the Santa Ana City Council. <br />Contract: A resulting contract may include any written material made as part of your proposal. This material <br />may include but is not limited to; answers to questions contained in this RFP letters, telegrams, facsimile and <br />product literature. <br />STATUTES AND RULES: The terms and conditions of the RFP, the resulting services, and furnishing of <br />commodities based on the RFP shall conform to the statutes, rules and regulation of the Federal Government, <br />State of California, City of Santa Ana and the Santa Ana Police Department. <br />ii <br />25J-89
