V. DEFINITIONS <br /> A. Accident <br /> Any incident which results in property damage, vehicle damage, and/or <br /> injuries that are determined to be PREVENTABLE will result in the loss of <br /> an award regardless of the cost incurred from the incident. NON- <br /> PREVENTABLE claims will not result in the loss of any award. <br /> B. Employee Injury <br /> Any injury which is determined to be related and results from a violation of <br /> a Company rule or regulation, and requires medical treatment beyond In- <br /> House first aid classification will not result in the loss of an award. This <br /> program in no way, restricts or is intended to restrict an employee from <br /> seeking medical attention when injured on the job. <br /> C. Safety Violations <br /> Employees observed engaging in unsafe work practices which violate <br /> Company Work Rules, Company Safety Rules, Department of <br /> Transportation (DOD Safety Regulations, or California Vehicle Code <br /> Regulations will receive a Safety Violation Notice. The issuance of three (3) <br /> Safety Violation Notices during the Safety Quarter will result in the loss of <br /> an award. Three (3) Safety Violation Notices will count as the equivalent of <br /> one (1) accident or injury, pertaining to the Company Disciplinary Policy. <br /> VI. ACCIDENT REVIEW BOARD <br /> Each vehicle incident or injury will be reviewed by the Accident Review <br /> Board. The Accident Review Board will consist of the Operations Manager, <br /> Safety Manager and immediate supervisor. The Board will meet once a <br /> week to review each incident and determine whether the incident was <br /> "PREVENTABLE" or "NON-PREVENTABLE". The Accident Review Board <br /> will forward its findings and recommendations to the Division President. <br /> Said recommendations will include measures to prevent the recurrence of <br /> similar incidents and appropriate disciplinary action will be taken when <br /> warranted. <br />
