Preventable Injuries Caused by <br /> Violation of Company Rules/Regulations Action <br /> 1st Injury Written <br /> 2nd Injury within any 7 month period 3-day Suspension <br /> 3rd Injury within any 7 month period Termination <br /> NOTE: Termination will depend on severity and circumstances of <br /> accident/injury. <br /> Progressive Discipline - Continued <br /> Chargeable Unsafe Work Practice Action <br /> 1st Chargeable, within any 7 month period Written Warning <br /> 2nd Chargeable, within any 7 month period Written Warning <br /> 3rd Chargeable, within any 7 month period 3-day Suspension <br /> 4th Chargeable, within any 7 month period Termination <br /> NOTE: Notices remain in effect for 7 months from date of issuance. <br /> See Minor Violation, on the preceding page. <br /> Reporting. Accidents or injuries must be reported to a supervisor immediately. Written <br /> accident/injury reports will be completed prior to the end of the work shift on the day of <br /> occurrence. Personnel involved in an accident or injury incident will be counseled on the <br /> determination of cause of the incident, and on measures to prevent reoccurrence. In <br /> cases where an employee's injury prevents an immediate written employee report and <br /> counseling, both will be accomplished at the earliest possible time. <br /> Employees involved in an accident/injury may be temporarily suspended with or without <br /> pay, pending investigation of the accident/injury. <br />
